Insurance & Fee Information

Co-pays, co-insurances and deductibles are due at the time of service. Non-payment of two (2) co-pays may result in the termination of services.

A $30.00 no show fee will apply for services canceled less than 24 hours in advance.

Fees will apply for "case management" services related to your case, such as maintaining professional license monitoring, custody related issues, social security disablitiy, educational assessment paperwork, or court/attorney related correspondence, etc.

The Billing Department is available Monday through Thursday 9AM to 7PM, and Friday 9AM to 4:30 PM, to discuss your financial responsibility regarding your services rendered at this facility.
