The Enrollment Process

You don’t need to know what level of treatment you need. Just call Mazzitti & Sullivan and request an assessment, and we will work with you to begin your recovery—wherever that beginning may be.

To become a new client at Mazzitti & Sullivan, simply call and ask for an initial evaluation. We accept most private and public insurance, so have your insurance card in hand when you call. You may specify the location where you’d like to be seen.

Your first appointment will last about an hour. A trained therapist will address your primary concerns and make recommendations, which may include scheduling appointments on a regular basis. The frequency of your appointments will depend upon what you and your therapist deem necessary to achieve the identified treatment goals.

To expedite your check in process, please print and fill out the New Client form and prior to your first appointment.

All information discussed, including the results of drug testing, is completely confidential and cannot be released until/unless a patient signs a release.
We regularly conduct drug and alcohol assessments for those who are mandated to obtain one. With your signed consent, we communicate with probation officers, lawyers and/or other collateral contacts to help you through your legal situation. If you must complete an educational class or receive treatment, we will design a treatment plan that will satisfy any court order or program requirements.
Whether you suspect your adolescent child may be using or your child has gotten into some legal trouble, we can help. Our locally renowned adolescent treatment programs are available at several locations; please call 1-800-809-2925. To start, schedule a drug and alcohol assessment for your adolescent child. During this appointment, a trained therapist will meet first with your child, and then the therapist will consult alone with the parent(s) or guardian(s) who attend the appointment. By the end of the assessment, the therapist will make recommendations for treatment. The recommendations may include ongoing individual counseling with weekly or biweekly appointments, an adolescent-specific treatment and educational group or a combination of both.
Cost of treatment varies depending on your medical insurance coverage. If you’d like to know how much your session will cost, the best way to find out is to call your insurance company directly; there is a phone number on the back of your insurance card.
We offer self-pay rates for evaluations and ongoing treatment. If you live in Cumberland/Perry, Dauphin, or Lebanon Counties, mention this when you call; you may also qualify for assistance.
